SECTION 1: General Participant & Program Authorization: As the parent or legal guardian of the minor camper named below, I hereby give full permission for my child to attend Lamoka Baptist Camp and to participate in all standard camp programs, events, recreation, and camp-sponsored off-site trips.
SECTION 2: Media & Promotional Release: I grant Lamoka Baptist Camp, its agents, employees, and assigns irrevocable permission and absolute right to capture, copyright, publish, use, and redistribute photographic images, video footage, audio recordings, or likenesses of my child taken while at camp. These assets may be used for promotional, marketing, advertising, web design, commercial, and archival purposes across any media platform (including print, digital, video, and social media) without prior notice, compensation, or inspection rights on my part.
SECTION 3: Dismissal Policy & Financial Responsibility: Lamoka Baptist Camp reserves the right to dismiss any camper from the premises at any time and for any reason, at the sole discretion of camp administration. Dismissal Costs: In the event of a camper dismissal (whether due to behavioral issues, homesickness, code of conduct violations, or medical necessity), parents or legal guardians accept full responsibility for immediately arranging and paying for all costs associated with picking up and transporting the camper from camp. Refund Policy: All camp fees and registration costs are strictly non-refundable after the camper has checked in on Monday morning. Any refunds before that point are at the discretion of the camp Administrator and the registration fee is always non-refundable.
SECTION 4: Food Service & Dietary Restrictions: Permission to Serve: Lamoka Baptist Camp provides meals and snacks during the camp session, and I grant permission for camp staff to serve these meals and snacks to my child. Allergies & Dietary Restrictions: Parents and legal guardians are strictly required to submit complete information regarding any camper allergies or medical dietary restrictions prior to arrival. However, by signing below, I acknowledge and accept that despite best efforts by camp food service staff, cross-contamination or contact with allergy triggers (including but not limited to nuts, dairy, gluten, and other allergens) may occur. I explicitly assume all risk of contact, allergic reactions, and associated medical consequences.
SECTION 5: High-Risk Activities & Zip Line Waiver: Camp programming includes high-adventure and physical activities, notably the Zip Line Course. By signing this agreement, I acknowledge and incorporate the specific rules, inherent risks, and liability protections of the Lamoka Baptist Camp Zip Line Course. Zip Line Rules & Safety: All participants must wear camp-provided safety equipment (no personal equipment allowed), remain connected to safety lines on elevated platforms, wear closed-toe shoes, secure long hair, carry no personal objects (phones, cameras, etc.), and follow all staff instructions. Crying or unwilling participants will not be permitted to ride. Inherent Risks: Participation in camp activities (including zip-lining, climbing, sports, and recreation) involves inherent and other risks ranging from minor scrapes, bruises, stings, and sprains to broken bones, neurological damage, emotional trauma, or even death. These risks may arise from natural environmental hazards (terrain, weather, trees, water), equipment operation or defects, or the negligence of camp staff or other participants. Assumption of Risk: I knowingly, freely, and voluntarily assume all risks—both known and unknown, inherent and uninherent—associated with my child's participation in all camp activities.
SECTION 6: Morality Statement: Lamoka Baptist Camp is a Christian ministry operating under traditional biblical standards. By signing below, I acknowledge and agree to support the standards outlined in the Lamoka Baptist Camp Morality Statement (Version 2020A): 1) Sexual Identification: God's will as evidenced at birth serves as the determining factor for sexual identification; all boys born as boys and all girls born as girls are required to conduct themselves accordingly. 2) Sexual Standards: All sexual activity outside of traditional marriage is considered sinful. 3) Definition of Marriage: Marriage is strictly defined as being between one man and one woman. Any violation of these behavioral and moral standards by a camper or participant immediately disqualifies them from participation in activities on campus, subject to immediate dismissal without refund.
SECTION 7: Comprehensive Release, Indemnification, and Hold Harmless: To the maximum extent permitted by New York law, I hereby release, waive, forever discharge, and covenant not to sue Lamoka Baptist Camp, its board members, officers, agents, employees, and insurers from any and all liability, claims, demands, or causes of action arising out of or connected to my child’s attendance, participation in camp activities, food consumption, or dismissal. I further agree to defend, indemnify, and hold harmless Lamoka Baptist Camp from any and all losses, claims, damages, attorney's fees, and costs incurred as a result of my child’s actions, negligence, or participation, including any third-party claims. Furthermore, I agree that any claim or dispute arising from or related to this agreement or my child’s attendance at camp shall be governed by the laws of the State of New York, and any legal action must be brought exclusively in a court located within the State of New York.
