RULES and LIABILITY WAIVER PLEASE read the following rules and waiver. By completing and submitting this form, you (or the parent of those under 18) agree to the following rules and waiver. 1) All staff must be on sides (girls on girl's side and guys on guy's side) by 9pm, in cabins by 9:30pm, and turn the lights out at 10:00 pm 2) Staff may only enter the following buildings: their cabin, restrooms, and kitchen/dining hall (and only for food that is permitted for consumption by weekend staff). 3) All staff staying on campus Sunday nights must stay on the main campus at all times, and cannot leave after arrival without permission of the Administrator (and their parents if they are under 18) 4) Staff are not permitted to be in isolation with anyone of the opposite gender (either inside or outside). 5) All staff staying on campus must notify the Administrator the day before, or on the day of, to remind him they are staying. This must be done every weekend they plan to stay. 6) All staff (and the parents of staff under 18) must recognize and sign off on the fact (by submitting this form) that there is NO supervision on campus on Sunday nights/weekends. Any violations will result in either immediate expulsion and/or inability to stay Sunday nights in the future and/or dismissal from the camp staff By completing and submitting this form, I hereby voluntarily release and forever discharge Camp Lamoka and each of their agents, employees and/or insurers from any and all liability, claims, demands, actions or rights of action, which are related to, arise out of, or are in any way connected with the stay of the undersigned, including, but not limited to the negligent acts or omissions of Camp Lamoka, its agents and/or employees, and of all other persons or entities, for any and all injury, death, illness or disease, and damage to my property. To the extent permitted by law, I hereby agree to and shall at all times defend, indemnify and hold Camp Lamoka and its officers, agents and employees, wholly harmless from any and all losses, costs (including court costs and reasonable attorney’s fees), expenses, penalties, response costs, claims, demands, suits by any person, persons, or entities whether or not frivolous), injuries, damages or death and other liabilities of whatever kind or nature, arising out of (directly or indirectly), connected with, incident to or resulting from the undersigned stay at camp over either the weekend or Sunday night(s) specifically.
